Dialysis Transportation Partner for Social Workers & Case Managers | Atlanta & DeKalb
Heart2Heart works directly with nephrology social workers, dialysis clinic staff, and hospital case managers to set up standing recurring transportation for patients across Atlanta, Decatur, and DeKalb County. We do not accept Medicaid. Heart2Heart bills insurance for medically necessary transports such as dialysis, wound care and emergency-department related trips. Coverage depends on the patient's plan and is never guaranteed, and we do not bill Medicaid. Rides booked and paid for directly on this website are paid by card at booking.

What you actually need from a transportation vendor
You are not shopping for a ride. You are trying to close a gap in a care plan without adding a weekly follow-up task to your caseload. The vendor either holds the schedule or it becomes your problem again in two weeks.
Heart2Heart is built around recurring medical transport rather than on-demand trips, which is why standing dialysis schedules are our core work rather than an accommodation. Below is what that means in the terms that matter to your job.
How we work with clinics
Standing recurring schedules
We build the schedule around chair time, not the other way around. Give us treatment days, chair times, and the patient's address, and the trip repeats weekly without rebooking. Chair-time changes, added sessions, and temporary holds for hospitalization are handled by a single call to the office.
Reliability framing
We do not publish a headline on-time percentage, because a number without a definition is marketing. What we will do is tell you exactly how we measure it, report on your referred patients on request, and flag a pattern before it becomes a missed treatment. Return legs are planned with margin for sessions that run long, and door-through-door assistance is standard in both directions — which is the piece that prevents post-treatment falls at the front door.
Payment handling
Heart2Heart bills insurance for medically necessary transports such as dialysis, wound care and emergency-department related trips. Coverage depends on the patient's plan and is never guaranteed, and we do not bill Medicaid. Where a plan-arranged ride falls through, we can begin transport as private pay the same day so the patient does not miss treatment.
We do not accept Medicaid under any circumstances. If a patient is a Georgia Medicaid member, their rides belong with the regional NEMT broker, and we would rather tell you that on the first call than three weeks in.
Vehicle capability
Wheelchair-accessible vans with proper securement for patients remaining in their own chair, and stretcher transport with two attendants for patients who cannot sit upright. One companion rides free, space permitting. Lift Assist is available at $75 where a patient must be physically lifted, and Extra Stops add your vehicle's base fare per stop (wheelchair $180, stretcher $420, BLS ambulance $450) plus the added mileage.
Direct scheduling line
Referring staff get the office line rather than a consumer booking form — call and ask for recurring dialysis scheduling. You can also send a referral through the form below and we will call your facility back.
The DeKalb dialysis corridor
Our base is on Candler Road in Decatur, which puts us inside the county's densest concentration of outpatient dialysis. We run standing schedules to and from clinics around the 30032 and 30317 ZIP areas, near Emory Decatur, and throughout Decatur, Candler-McAfee, Belvedere Park, Avondale Estates, Scottdale, Clarkston, Stone Mountain, Tucker, Kirkwood, and East Atlanta, plus the city of Atlanta.
Short local runs are where standing schedules hold up best, and it is why our reliability on this corridor is the thing we would ask you to test us on first — start with one or two patients and expand if we earn it.
Refer a patient
Use the form below to start a referral. Please do not include patient health information: no diagnoses, no clinical notes, no medical record numbers, no dates of birth. Tell us the mobility level and schedule needs in general terms and we will collect the rest directly from the patient or through your normal secure channel.

Heart2Heart Medical Transport does not accept Medicaid under any circumstances. Dialysis is the only trip type billed to insurance (commercial, Medicare, or a qualifying Medicare Advantage plan), and prior authorization must be on file before the trip. All other transports are private pay. If you are experiencing a medical emergency, call 911.
